This is the form needed to be placed at the back of your artwork for every one of our exhibits. This form has to be filled up in duplicate, the top half of it affixed behind each individual piece presented for jurying, and the bottom half for RAA administration purposes. Print as many forms as the number of pieces you may be presenting.
